This is a single sheet 28 X 17 inches (71 X 43.5 cm) printed on one side only. It has instructions on how to crochet 3 different potholders: An Old-Fashioned Girl; A Square; A Hexagon using 3 strand Mercerized Crochet Cotton.

The pattern sheet is folded and measures 7-1/8 X 4-3/8 inches (18 X 11 cm). It weighs ½ ounce (15 grams).

I am not sure of the manufacture of this pattern. It might be Susan Bates as it mentions using Susan Bates Fashion Tone Yarn.
