We are not the original owners so we REALLY do NOT know much about this lot
such as their condition, completeness, etc.
However we will tell you what we see :-)
The label on the top says:

In the back are 3 round ports.
The middle one is much smaller than the ones on its sides.
The two larger ones have plastic caps on them.
In the front are 2 rows of 6 ea: screw terminals.
The top 6 say: -V +V DA OH DT LC1
The bottom 6 say: SH SG CCT R1 DR LC2
To the right of that section is a wire coming out that is attached to the
GDC Part No. 700-026-001 REV. H
On that piece are screw terminals that say 1R 2Y 3B GRD.
The above wire is attached to 1R and 3B
The GDC Part No. 700-026-001 REV. H says:
INPUT 120 VAC 60HZ 0.15A CLASS 2 TRNSF.
OUTPUT 18.5 V 12 VA
