Problem with International Shipping in Bulk Uploader

I just did a test run with the bulk uploader and received a FAILED report: "International Shipping Please add at least one country to international postage"

I want to format my shipping costs as:
Canada
Standard Mail: $8.50 for the first CD, $2.50 for each additional.
Priority Mail: $22.00 for the first CD, $2.50 for each additional.

Rest of the World
Standard Mail: $11.00 for the first CD, $4.00 for each additional.
Priority Mail: $26.00 for the first CD, $4.00 for each additional.

So I entered:
International Shipping
04=8.50+2.50, ca:06=22.00+2.50, ca:04=11.00+4.00, eu, as, sa, af, oc:06=26.00+4.00, eu, as, sa, af, oc:

Can anyone help please help me figure out what is wrong with my code?

Contact support. My hunch is that the +2.50 additional shipping cost could be the problem
