From the Help files:

After upgrading to Seller+ Lifetime, simply contact us via our contact form on the Helpdesk index page with your username.

It doesn't place a time limit on how long after upgrading you have to apply. As long as the offer was in place when you upgraded you should be eligible for a T-shirt.

Even if the offer was not in place at the time you upgraded, I can't see them being that tight and would more than likely send a T-shirt to anyone with a Seller+ Lifetime account.

Ask; the worse they can do is say no.
