The rear of the pattern is missing but I found the pattern description on line:

Simplicity 3094, ca. 1959; Toddler’s Dress or Jumper and Blouse-Slip: Dress-jumper has square neckline, back button closing and a flared skirt. V. 1 and 3 have patch pockets. V. 2 and 3 are trimmed with rick-rack. V. 3 tie ends are self fabric. Blouse-slip V. 1 and 2 have high round neckline, set-in sleeves, back button closing and flared skirt. V. 1 3/4 length sleeves have self wifles. V. 2 has short sleeves and is lace trimmed.

The pattern pieces are in good condition, only the 2 pieces for the skirt have been used.

The pattern envelope is in poor condition, the rear of the envelope is missing, the front is in very poor condition with rips all around the edges. (see Pictures).
