It is 1 package of 4. These are in the Black with Colorful plastic rhinestone gems style pictured.
Packaging from 2012. I also have newer packages from 2017 now. You may get either, or specify a preference.
I noticed that one of them is missing a gem. It looks like that is a manufacturing flaw, and there may be others like that, but I never actually did find another like that since.
